Near-accidents and work-related injuries
Instructions for reporting and investigation (within KI) and reporting to Swedish authorities.

A manager/other responsible person must notify the Swedish Work Environment Authority without delay (the same day) of any death or severe injury that has occurred in connection with the performance of work or studies. The same applies for severe near-accidents. This is to be done on www.anmalarbetsskada.se. Negligence to do so may result in a personal criminal liability, according to 3 Chap. 3a § Work Environment Act.
Managers with staff liability are obliged to report work injuries to Försäkringskassan, according to the Social Insurance Act. This is also done on www.anmalarbetsskada.se.
We observe something (physical, technical, chemical, biological, organizational or social) that could lead to a near-accident or work-related injury.
Unwanted and often sudden incident at work/studies that could have caused physical or mental/psychological ill-health
Work related injury
A term which includes accidents when travelling to and from work, accidents with personal injury/death and work-related illness/other harmful influence from work/studies. The latter includes work-related infections (e.g. after a needle stick injury).
A work-related injury is often a physical injury but the term also encompasses ill-health/illness due to organizational and social factors in the work environment. Bullying or high degree of work load may cause ill-health as well as violence or threats at work/in the studies.
Report/notification of near-accidents
Notification of near-accidents to the Swedish Work Environment Authority
If a near-accident is serious, that is to say the incident could have caused grave personal injuries (physical or mental) or death, the immediate manager/other responsible person at KI is obliged to notify the Swedish Work Environment Authority without delay (the same day) on www.anmalarbetsskada.se. This demand on notification includes both employees and students at KI.
Reporting of near-accidents within KI
All near-accidents, both serious and less serious, shall be reported to KI incident reporting system. Risk observations may also be reported as near-accidents in this system.
All co-workers and students with a KI log-in shall report near-accidents in the system. In addition to reporting to the system, information also shall be given to other functions dealing with work environment issues at the department/equivalent such as HR/personnel, the chair of the work environment group, safety representatives and the Student Health Centre (applies to students and PhD students).
The immediate manager/other responsible person shall investigate the contributory factors causing the near-accident and then document the investigation and actions, already carried out or planned, in the same system. Safety representatives, student safety representatives, the local work environment group and others shall have the opportunity to participate in the work with corrective and preventive measures – so that similar near-accidents never will occur again.
Students can contact the Student Health Centre, should they have questions on the reporting of near-accidents.
Report/notification of work-related injuries
Notification of work-related injury to the Swedish Work Environment Authority
If a work-related incident has caused grave personal injuries or death of an employee/student, the immediate manager/other responsible person is obliged to notify the Swedish Work Environment Authority on www.anmalarbetsskada.se without delay (the same day). The same applies if a group of employees/students was affected on the same event (then the injuries don’t have to be grave, the severity lies in the fact that more than one person was injured in the same incident).
A travel accident to and from work/studies is exempted from this rule (but travel accidents during work/studies are comprised).
Notification of work-related injury to Försäkringskassan, AFA insurance/Kammarkollegiet
If the work-related injury/ill-health has led to sickness leave or it is likely that it will lead to disorders later on, the immediate manager shall notify the work-related injury to Försäkringskassan on www.anmalarbetsskada.se. The same applies for accidents when travelling to and from work or studies and in the event of an employee/student’s death in the work or studies.
A copy of the notification shall be duly registered and filed at the department/equivalent according to the handling instruction for Karolinska Institutet documents. Note that the affected employee/ student also shall receive a copy of the notification. Then the affected person can apply for compensation at Försäkringskassan (only in Swedish).
In addition the affected person can also apply for compensation from AFA försäkring (employees) and Kammarkollegiet (students). The employer shall inform employees/students on this. The AFA-insurance can also cover compensation for personal belongings (for example a pair of spectacles) that were broken in the accident.
Reporting of work-related injuries within KI
Work-related injuries/ill-health also have to be reported to KI incident reporting system. All co-workers and students with a KI log-in shall report work-related injuries/ill-health in the system. If a notification to Försäkringskassan already has been done, a digital copy of the notification may be added in the system in order not having to fill in all facts one more time.
The immediate manager/other responsible person shall investigate the contributory factors causing the work-related injury/ill-health and then document the investigation and actions, already carried out or planned, in the same system. The affected person, safety representatives, local HR/personnel, local work environment group and Student Health Centre shall have the opportunity to participate in the work with corrective and preventive measures – so that similar injuries/ill-health never will occur again.
Possible compensation for medical attendance and medicine is handled by the regular payroll administrator at the HR Office. See Chapter 7 in the Personnel handbook.

Special routines for puncture injuries
Puncture injuries, employees
Immediately contact Avonova occupational health services to have a "blank/zero" blood test. Inform your immediate manager of what has happened. The manager will, after consulting with Avonova, if needed, determine if the incident should be reported as a work-related injury at www.anmalarbetsskada.se or only be reported to the KI incident reporting system.
Punctures or lacerations shall be reported both to the Swedish Work Environment Authority and Försäkringskassan if:
- known or strongly suspected contaminated blood is involved,
- there has been a contact with materials/patients/persons where it is a known or suspected serious contamination.

Puncture injuries, students
See the Student Health Centre's website regarding how to act in the event of a puncture injury or other work-related injury. Here, a decision is taken in consultation with the Student Health Centre regarding whether the puncture injury shall be reported both to Försäkringskassan/Kammarkollegiet and the Swedish Work Environment Authority.
All puncture injuries shall also be reported to KI incident reporting system.
Take preventative measures immediately after a risk observation to reduce the occurrence of near-accidents and work related injuries.
This creates a safe and healthy working environment, physically and mentally.
