Registration numbers and forms

Depending on your application or ongoing project, different forms may be required. These forms must comply with legal and funding agency-specific requirements. Additionally, some forms may need to be renewed annually throughout the project period

EU Registration numbers and forms

EU Form 1 is needed at the application stage, whereas EU Form 2 has to be signed in conjunction with relevant agreements and usually happens later in the process.

  1. Application assurance form – must be completed and signed by the Head and Administrative head of the department, as well as the PI before the application can be submitted
  2. Researcher consent form – must be signed by everyone at KI working within the project, and states that the person signing has read the related agreement(s) and is aware of ones responsibility
  3. KI Time sheet template - should be completed by everyone at KI working on the project. Your time sheet needs to be printed, dated, and signed monthly 
  4. Documentation of purchases outside a written agreement - should be filled in for every purchase outside a written agreement, regardless of the amount, in order to comply with EU’s rules to prove “best value for money”

US forms 1-4 are needed at the application stage. Forms 3-4 are part of the annual update process and other forms are used when relevant.

  1. Application assurance form – must be completed and signed by the Head and Administrative head of the department, as well as the PI before the application can be submitted
  2. PI assurance form – signed by the PI and ensuring that all legal and funding agency requirements for the project will be met
  3. Financial conflict of interest (FCOI) – should be completed by all key personnel in the project and has to be renewed once a year, these are not project-specific reports
  4. Effort reporting form – should be completed by all key personnel in the project, and has to be renewed once a year – these are not project specific though, and the same report can be used for different projects
  5. Ethical assurance form – needed when human subjects are part of the project, and assures that proper ethical approval(s) have been received (please contact compliance@ki.se when applicable)
  6. Animal assurance form – needed when animals are part of the project, and assures that proper certificates and ethical approvals are in place (please contact compliance@ki.se when applicable)
  7. Researcher consent form – must be signed by everyone at KI working within the project, and states that the person signing has read the related agreement(s) and is aware of ones responsibility
  8. Data access request form – must be completed and signed before any data access requests can be processed
  9. Time sheet - should be completed by everyone at KI working on the project, and be printed, dated, and signed monthly

Data access requests (DAR)

There are several databases open to researchers, for some access is managed via CDO (Compliance & Data Office). Typical examples are dbGaP and NIDDK.

Before you make a Data Access Request you need to submit a Data Access Request Form to compliance@ki.se.

For access to dbGaP, you also need an active eRA account, as a PI, before you can submit an access request. Please contact compliance@ki.se if you need help creating one
